Nelly there are some truly lovely comments in your EPAD from your practice assessors who have
identified that you are polite, hardworking, always punctual and go the extra mile to provide good
quality care to your patients – your PA even identified that you have gone beyond expectations. What
is more – you seem to have developed excellent relationships with those staff who work with you –

– Nelly further developed her knowledge and understanding of the CAMHS service and the referral / criteria processes including service threshold and risk rating through assessing, discussion and observation.

 

– Nelly participated in the Choice assessment’s, initially observing choice assessments, following this Nelly participated in the assessment and the written choice report.

 

– Through the choice assessment Nelly has further developed her knowledge around presenting difficulties in young people such as self-harm, low mood, autism and trauma.

 

– Nelly has developed her knowledge around the roles of other professionals within the professional network and demonstrated this knowledge through verbal and written reflections around the MDT roles.

 

 

Skills

 

Nelly has developed her skills in report writing (completing choice reports).

 

Nelly demonstrated active listening skills throughout the choice assessments and when accessing team meetings / professional meetings etc.

 

Due to the lack of learning opportunities within CAMHS, Nelly has had to carry out self directed learning / projects. Through this, Nelly demonstrated organisational skills as well as being self-disciplined, self-engaged and self-reflected.

 

For a 1st year student, I am very impressed by the high level of Nelly’s skills and overall performance within her placement.

 

 

Attitudes and values

 

Nelly has a positive attitude to learning and continues to ask questions in relation to young people and mental health.

 

Nelly shows empathy and compassion within her role and it is evident she practices the nursing values including being respectful, open, compassionate and working collaboratively

   Midpoint Since starting placement with the Child and Adolescent Mental Health Service (CAMHS), Nelly has developed her knowledge and understanding of the CAMHS service.

Nelly shared with me on starting placement at CAMHS, that she had limited experience and knowledge around the mental health difficulties presenting in children and young people.

Since starting placement, Nelly has observed and developed her knowledge by observing a range of professionals within the CAMHS team

Nelly has also gained a greater understanding about neurodevelopmental conditions including Autism and the impact autism may have on young people’s mental health.

Skills

 

Nelly has had the opportunity to observe initial assessments within CAMHS and has developed her skills in assessment and safety planning.

Nelly has demonstrated her assessment skills by providing written reports outlining the information taken at assessment which includes, background information, current presentation, family / developmental history / school, friendships / risk / summary / recommendation and plan going forward.

Nelly has a greater understanding since starting her placement at CAMHS around the complex mental and emotional needs of children and young people and has been pro active in increasing her knowledge and skills to understand these complexities.

Nelly has a positive attitude when working with children and young people. Nelly is calm and approachable in her manner. Nelly is conscientious in her work and is open to learning new ideas / approaches to best meet the needs of young people.

Knowledge

 

Student nurse has achieved all her initial set goals for Colorectal ward in Gladstone 2 ward and has gone beyond in her learning. She has knowledge in the different medical problems of colorectal i.e Chrons, Total pelvic exenteration, Examination under Anathematic, Colorectal cancer, stoma reversal, Vac therapy dressing therapy. Knows the different types of lines i.e Hickman line, picc line, mid-line, cannula etc and its importance in medicine administration. Has knowledge in health and safety and infection control. She knows her limits, able to escalate concerns to her mentor. Always present at board round to learn about patient care plan for the day and its implementation.

 

 

Skills

 

Student nurse has grown, developed confidence and demonstrated skills in:

 

Personal care, vital signs, bed side management, stoma, drains and catheter care, documentation, communication, telephone mannerism, taking hand over, pain control management, fluids and medicine administration, escorting patient to theatre, aseptic technique.

 

Her documentation and communication skills are outstanding.

 

 

Attitudes and values

 

Student nurse attitude is outstanding always polite and courteous, team player, focus on patient centre care. Clean and tidy in all her shift, adhere to dress code, communicate well. Demonstrated good attitude and adhered to the trust values, very respectful and always goes the extra mile in helping other in the ward. She will be an excellent nurse.

Knowledge

 

Student nurse has demonstrated knowledge in the colorectal care ward through learning from her mentor, team leaders, care team and multi-disciplinary team. Student nurse has gained and shown knowledge in her writing, know how to write care notes after giving care, has knowledge of the importance of taking hand over before taking over patient care, understand the layout of the handover documentation. Student nurse will always asked questions in order to gain clarity in anything she did not understand. Has knowledge in the basic process of medicine management and has administer medication under supervision, has given dalterparin injection under supervision. Student has knowledge in doing patient vital signs (observation), she knows the importance, able to chart the scores also able to escalate news score. She has learnt the importance of Neuro observations, she has knowledge of Patient Control Pain management (PCA). She has some knowledge of why patients are given fluids to prevent dehydration and to correct patient Electrolyte. She knows how to write and balance patient fluid chart. She has learnt telephone mannerism and knows how to bleep any multi-disciplinary team and communicate concerns. She has knowledge of admission and discharge procedure.

 

 

Skills

 

1. Wound dressing and Aseptic practices: She know the importance of Aseptic techniques to prevent infection.

 

2. Documentation: Her documentation and writing skills is outstanding. She is a quick learner.

 

3. Doing patients vital signs and Diabetic monitoring: she is quite confident in doing vital signs and able to escalate scores to nurse in charge.

 

4. Personal Care: She has demonstrated skill in bedside nursing by independently given personal care patiently, confidently and thoroughly.

 

5. Skin Assessment and documentation: She is observant, she will report skin concerns and document it.

 

6. Medication Management: She understand the drug chart and its importance and has physically administered medication and given dalterparin injection understand supervision.

 

7. Drains, Stoma, catheter care management: She understand the different drains in colorectal ward, she understood the different stomas and the different catheter care. She has confidently demonstrated skills in these areas.

 

8. Escorting Patient to Theatre: She knows the requirements of escorting patient to theatre for operation and has physically escorted couple of patients under supervision.

 

9. Health and Safety: She understands the importance of maintaining health and safety within the ward.

 

10. Communication skills: She has good communication skills. A team player, has collaborated well with all multi-disciplinary teams

 

 

Attitudes and values

 

Student nurse is a very humble learner, polite, has good attitude, never complaint no matter how busy the ward is. She has patient centre care skills. She is always punctual at work, never absent. Her dress code is clean and good, hair always tied back. Her behaviour and mannerism are outstanding always willing to learn more and go the extra mile. I have watch her grow in a short space of time especially in her writing skills and patient centred care skills. She works as a team always helping others fs asked to help. She will be an outstanding nurse when qualify.
