Title of Learning Activity – Reflection on post Handover and Patient blood Monitoring

As a nursing associate student, after the morning handover i shift into patient care smoothly. the handover provided crucial updates about the patients, allowing me to prioritize tasks. one of my key responsibilities was to perform blood monitoring and assess the vital signs of a patient.

me and my colleague approached the patients with confidence, explained the procedure to them and ensured they were comfortable. i accurately recorded their blood pressure, heart rate, temperature and oxygen saturation, ensuring that all measurements were within the normal range. i documented the findings in patient’s chart and informed the result to the nurse in charge.

this experience highlighted the importance of clear communication during hand over, as well as attention to detail in performing clinical duties.  it also helped me strengthen my skills in managing patient care  and integrating my theoretical knowledge into practical tasks as a nursing associate student.
