Title of Learning: Ward Handover
This morning handover, an issue was raised regarding a patient’s conveen frequently coming off when they sit in a chair. this has been on going problem, causing discomfort for the patient and concerns about hygiene and skin integrity. as a nursing associate student ,i was tasked with understanding the underlying cause and working with the team to find solution.
the issue highlighted the importance of proper device and patient positioning. if the conveen continues to slip, it could lead to increased risk of infection or skin breakdown. we realized the need of continues monitoring and ensuring they are securely applied. it also emphasizes communication and teamwork, as it was important for the staff to relay this informationĀ during the handover and consider multiple factors like mobility and chair positioning.
moving forward we will ensure to double check the application of the conveen when assisting patients, especially when they are being moved or seated or transfer to bed using hoist. we will also communicate any concerns to the senior staff to prevent further complications. as nursing associate student, i will seek guidanceĀ on best practice for device management and considerĀ other solution if needed, such as adjusting the type of adhesive used or other type of conveen.
