Title of learning: Ward Handover

As a nursing associate, I am actively involved in ensuring smooth care transitions through effective handovers and tailored care plans. in reviewing the care plan for a patient with left ankle wound and left hip wound, we identified the need for a balance between mobility and rest to promote recovery while minimising discomfort. the updated plan includes a morning session in the chair for 2 to 3 hours, allowing the patient to experience mobility, preventing stiffness, and engaging socially. during handover, i communicate these details to ensure all team members understand the importance of this mobility period for the patient’s physical and emotional well-being.

following the morning session, the patient rests in bed for 2 hours, which gives the ankle and hip wound adequate relief from pressure. this rest period balances mobility with the patient’s recovery needs, reducing the risk of strain and complications. my role as nursing associate involves monitoring for any signs of pain or fatigue during these changes, and during handover, i share my observation and concerns  to ensure continuity of care. compassionate communication in handovers is essential to keep everyone informed and aligned with the care plans goals.

in the afternoon, the patient has a second session in the chair to maintain mobility without compromising recovery. this structured plan supports patient-centred care, which is a core aspect of my role. during handover, i emphasise the patient responses to the plan, noting any adjustment needed based on their comfort and progress. this reflective handover practice helps the team responds effectively to the patient’s needs, creating a compassionate approach to supporting their healing.
