Title of Learning: Musculoskeletal System

As nursing associate trainee i recently learned about musculoskeletal system, which include bones, muscles, tendons, ligaments, and joints. this systems provides structure, protects vital organs, facilitates movement, and support the body. the lesson covered the function of bones ( example protection and mineral storage), the types of muscles skeletal, smooth, and cardiac. and the importance of  joints in enabling mobility.we also discussed common condition like fractures, arthritis, and muscular   dystrophy and their impact on patient care.

This lesson emphasised the importance of understanding the musculoskeletal system in nursing practice, especially in assessing mobility and preventing complications like pressure ulcers muscle atrophy in immobile patients. it also reinforced the need to handle patients carefully during transfers or rehabilitation to avoid causing injuries. knowledge of this system helps me identify signs of musculoskeletal issues, such as joint  swelling, muscle weakness, or bone deformities, and contribute to effective care plans.

I will focus on applying this knowledge in clinical practice bye observing patients’ mobility. identifying risk of fails or injuries, and supporting rehabilitation activities. I plan to improve my understanding of assisting with physiotherapy exercises  and using mobility aids effectively. additionally, I will further explore conditions like osteoporosis and arthritis to better understand their management. continuous learning and practice will enhance my ability to provide safe patient centred care.

Title of learning: Digestive System

Last Wednesday, I had a lesson on the digestive system, I learned about its anatomy, physiology, and the processes involved in digestion, absorption, and waste elimination, key components such as the roles of the  stomach, small intestine, liver, and pancreas were covered. This knowledge is essential to understanding how the body obtains and utilises nutrients, which directly impacts patient care, especially when managing nutrition or gastrointestinal issues.

This lessons highlighted the importance of recognising sign of digestive disorders in patients, such as malabsorptions, dehydrations or gastrointestinal blockages. for example understanding the digestive process helps me interpret symptoms like abdominal pain, changes in bowel habits, or vomiting, and identify potential issues like bowel obstruction o0r gastritis, bit also connects to nursing pwracticessuch as administering enteral feeds or monitoring for complications like aspiration in patients on NG or PEG tubes.

I plan to apply this knowledge in practice by being more attentive to patients nutritional needs and gastrointestinal symptoms. I aim to enhance my skills in assisting  with feeding, monitoring for signs of malnutritions or dehydration, and supporting patients with digestive disorders. I will also review cases in my placement to connect  theory to practice, such as assessing care plans for patients with Crohn’s disease or GERD. continuing professional development in this area will improve my ability to provide effective, holistic care.

Title of Learning: Percutaneous  Endoscopic  Gastrostomy (PEG)

Today I observed and assisted with a Percutaneous  Endoscopic Gastrostomy PEG tube change. This involved ensuring the patient was comfortable, preparing the equipment, and maintaining the aseptic technique to minimise the risk of infection. I learned  the importance of checking the condition of the stoma site and the proper insertion and securement of the new Peg tube.

this experienced reinforced the critical role of maintaining patient safety and comfort during procedures. I realised the importance o9f effective communication  to explain the process to the patient, even if they were non verbal, to ensure dignity and reduce anxiety. it highlighted my responsibility to observe closely, document accurately, and adhere to guidelines such as the NMC code and infection control protocols. the hands on experience improved my understanding of PEG management and increased my confidence in supporting such procedures in the future.

I plan to build on this learning by reviewing the hospital’s PEG care guidelines and attending further training if available. I will also seek opportunities to observe and assist  with similar procedures to enhance my competence. i will reflect on how to better involve patients in their care,  ensuring they feel  informed and supporte3d throughout. this aligns with my goal of providing safe, compassionate, patient centred care.

Title of Learning: Reflection on Reading About Percutaneous Endoscopic Gastrostomy [PEG]

Today I  learned about percutaneous Endoscopic Gastrostomy [PEG], a procedure used to provide long term nutrition for patients who cannot eat or swallow effectively. it involves inserting a feeding tube into the stomach through the abdominal wall, guided by endoscope. This procedure is often used for patients with swallowing difficulties caused by conditions like stroke or neurological disorders. Reading about this topic helped me understand its importance in maintaining proper nutrition and hydration in patients who are unable to consume food orally.

I found interesting was the focus on patient safety during and after the PEG procedure. Although it is minimal invasive complications such as infection, tube blockage, or accidental removal can occur if not managed correctly. I realised the importance of strict hygiene and routine care of the PEG site to reduce these risks. as a nursing associate student, this highlighted the role i will play in monitoring complications, educating patients, and ensuring the tube functions properly.

This ready also made me think about the emotional and psychological aspects of PEG care. For many patients, having a feeding tube can feel invasive or distressing. Proving reassurance and explaining the process clearly can help reduce their anxiety. I also considered the importance in involving patients and their families in discussions about care routines, as this can empower them to feel more confident in managing the tube.

This learning experience emphasised the need for a holistic approach to PEG care. it is not just about managing the feeding tube, it is also about addressing the patients physical, emotional, and social needs. I plan to build on this knowledge by practicing PEG care under supervision and learning more about supporting patients and families in adapting to life with a feeding tube. This will help me deliver safe, compassionate, and effective care in my future practice.

Immune and Lymphatic System

Title of Learning: Immune and Lymphatic System

Reading about the immune and Lymphatic systems has greatly enhanced my understanding of the body’s defence mechanisms. As a nursing associate student, I found it insightful to explore how these system work in tandem to protect against infections. the immune system  is responsible for detecting and responding to harmful pathogens, while the lymphatic system plays a vital role in fluid balance and waste removal. this foundational knowledge is critical in my role, where i support the body’s natural defences in promoting patient health.

Throughout this reading, I gained a clearer understanding of the roles of different white blood cells, each playing a unique part in the immune response. some cells actively attack invaders, while others retain a memory of pathogens to provide quicker responses in the future. This insight is valuable in clinical practice, especially when interpreting blood results where high white cell counts may indicate infections. recognising these indicators early can guide timely interventions, which is an important skill in patient care for a nursing associate.

I also learned about lymph nodes and their function in filtering harmful substances from the lymphatic system. when lymph nodes swell, its often sign of infection, something that i can observe as part of my patient assessments. additionally, understanding the movement of lymph fluid highlighted the importance of patient hydration, as adequate fluid intake support the lymphatic system’s ability to drain waste effectively.

reflecting on this knowledge, i see how essential it is for me to be observant and proactive in spotting signs of infection or immune responses. this reading has given me a deeper appreciation of how i can contribute to patient well-being by monitoring these vital signs and communicating changes promptly to the healthcare team. it has strengthened my commitment to holistic, patient centred care and better equipped me for my role as a nursing associate.

Reflecting on my recent lecture about the renal system, I’ve gained a deeper understanding of how this complex system plays a crucial role in maintaining our body’s health. the kidneys, being at the core of this system, work tirelessly to filter blood, remove waste, and maintain a balance of electrolytes and fluids. it was fascinating to learn how these bean shaped organs contribute to homeostasis by controlling essential functions, like blood pressure and pH balance, which are vital for overall well being. As a nursing associate, understanding these functions helps me appreciate the impact  that kidney health has on a patient’s entire body.

one key takeaway from the lecture was the concept of how waste is managed in the body. the kidneys filter out toxins and waste products from the bloodstream, sending them out as urine. when i consider patients who have compromise kidney function or conditioning like chronic kidney disease, i realise how challenging it must be for them to maintain this balance. this has helped me develop a sense of empathy for those dealing with renal issues, as they may feel exhausted or restricted due to their body difficulty in clearing  waste effectively.

The lecture also highlighted the importance of hydration and how it directly affects kidneys filter more efficiently, reducing the risk of kidney stones and infections. in practice, encouraging patients to drink enough water or recognise signs of dehydration is something i see as essential, especially for vulnerable individuals like the elderly or those with existing kidney issues. understanding the link between hydration and kidney health allows me to play a more supportive role in promoting good habits in patients daily routine.

This lecture has expanded my awareness of how critical kidney health is to overall wellness by understanding the renal system, I can better support patients with kidney conditions, whether by ensuring they follow their fluid intake recommendations or by recognising early signs of imbalance. this knowledge is empowering, as it enables me to offer more effective care and to advocate for small but impactful lifestyle changes that help patients maintain their kidney health.

Ward Handover

As a nursing associate, I am actively involved in ensuring smooth care transitions through effective handovers and tailored care plans. in reviewing the care plan for a patient with left ankle wound and left hip wound, we identified the need for a balance between mobility and rest to promote recovery while minimising discomfort. the updated plan includes a morning session in the chair for 2 to 3 hours, allowing the patient to experience mobility, preventing stiffness, and engaging socially. during handover, i communicate these details to ensure all team members understand the importance of this mobility period for the patient’s physical and emotional well-being.

following the morning session, the patient rests in bed for 2 hours, which gives the ankle and hip wound adequate relief from pressure. this rest period balances mobility with the patient’s recovery needs, reducing the risk of strain and complications. my role as nursing associate involves monitoring for any signs of pain or fatigue during these changes, and during handover, i share my observation and concerns  to ensure continuity of care. compassionate communication in handovers is essential to keep everyone informed and aligned with the care plans goals.

in the afternoon, the patient has a second session in the chair to maintain mobility without compromising recovery. this structured plan supports patient-centred care, which is a core aspect of my role. during handover, i emphasise the patient responses to the plan, noting any adjustment needed based on their comfort and progress. this reflective handover practice helps the team responds effectively to the patient’s needs, creating a compassionate approach to supporting their healing.

In our recent practical lecture, we learned about taking NEWS for Patients, a critical tool that helps track patient health and detect early signs of decline. as a nursing associate student, i practiced measuring and recording essential vital signs, heart rate, respiratory rate, blood pressure, oxygen level and temperature. this practical experience was essential in understanding how each parameter fits into a score that indicates a patient’s current condition and potential risks.

reflecting on this, i can see how crucial the NEWS scoring system is for maintaining patient safety. recognising and responding to subtle changes in vital signs can mean the difference between early intervention and delayed care. this practice taught me that every reading matters and that accuracy is vital, as even small errors can lead to misinterpretations. understanding the significance of NEWS scoring has reinforced my sense of responsibility in providing attentive and proactive care.

I aim to strengthen my skills in accurately taking and interpreting NEWS scores. in practice, I’ll be thorough and consistent, making sure to communicate  any important findings promptly to the healthcare team. this experience will help me become more confident in monitoring patients and contribute to timely, informed decision-making as part of patient care. applying NEWS in this way will support my growth as a nursing associate and improve my ability to deliver safe and effective care.

In our recent lecture on developing professional relationship in nursing, we explored the significance of equality, diversity, and inclusivity in creating effective connections with patients and colleagues. as a nursing associate student, i learned how these values are essential not just for patient care, but for building a supportive and respectful work environment. the lesson highlighted that truly professional relationships rely on understanding and valuing each person’s unique experiences and perspectives, which in turn helps build trust foster a caring environment.

Reflecting on this i began to see how embracing inclusivity, and equality can directly impact the quality of care we provide. when we respect and celebrate each person’s identity and background, patients feel more comfortable, valued,  and understood. for me as a student, it emphasized the importance of being aware of my own potential biases and taking steps to ensure i am better prepared to support a collaborative and cohesive team, where every member can feel included and respected.

I want to bring these insights  into my everyday interactions with both patients and colleagues. i plan to be more intentional about listening to each individual’s needs and background, showing genuine respect, and working to break down any barriers to inclusivity. by practising this consistently, i believe i’ll grow as a compassionate and effective nursing associate, contributing to a culture of care that truly embraces diversity. this approach will not only improve patients relationships but also reinforce my commitment to ethical and professional standards that guide our field.

Title of Learning: Ward Handover

During the ward handover last Sunday, i noticed a significant staffing issue. we had very few permanent staff on the floor, with most healthcare assistants(HCAs) being new agency workers who were unfamiliar with the ward. this left only myself and one other permanent HCA who knew the routines and patient needs. as a result, we faced several challenges in delivering smooth and effective care, especially with the added pressure of orienting the agency staff.

The lack of familiar staff created gaps in communication and teamwork, as agency staff often needed guidance on tasks and procedures specific  to the ward. This slowed down our ability to provide timely care and increased the work load for the two of us who were familiar with the setting. additionally, it raised concerns about patient safety, as temporary staff might not fully understand the patients individual needs and the wards specific routines. This experience highlighted the importance of well structured handover and the impact of staff shortages on patient care care and staff morale.

Moving forward, i see the need for strategies to improve handover processes when there are high numbers of agency staff on duty.  i could suggest  creating a simple briefing sheet with essential ward routines and key patient information to help agency staff orient themselves quickly.

additionally, better communication with  limited permanent staff may help address these issues. as a trainee nursing associate, i feel this experience has strengthened my understanding of the importance of continuity and teamwork in patient care, and it motivates me to find ways to support and guide new or temporary colleagues more effectively.
