Within your reflection, describe the episode of care and how you assessed, delivered and evaluated care.

On this reflection, I will be discussing about personal care that I supported CPR patient while at the ward for my placement. This patient is bedbound that need her observations to be taken. I introduced myself and the purpose of the procedure explained to the patient. Patient consented for the visit and was supported to stripped wash and dress up. She was always anxious of fall, I had to communicate in low tone and clear manner for the patient to understand and assure her that she was safe. In support with her personal care, I exhibited dignity and respect by closing all the curtains and covering her body parts that were not attended to. Her observations were taken and documented. Although she was always anxious to fall I always had to show care, compassion, communicate and courage and assure her that she was safe and doing well.

What did you do well?

I Communicated in a clear low tone manner for the patient to hear and understand and was very patience with the patient.

What would you have done differently?

Will always try me best to escalate any issues first before it gets out of hand
