Within your reflection, describe the episode of care and how you assessed, delivered and evaluated care.

 

 

In this reflection I will be discussing about medication and ECG procedure that I supported while on placement in a cardia ward. This patient complained about chest pain in the morning and I was asked to do ECG for her in which I exhibited care and compassion towards her. Prior to this procedure she was administrated. Bisoplor medication beta blocker) and diver tics medication (furosemide) she was on. The nurse in charge took some blood from her and was given morphine to reduce the pain. Her observation was also monitored and the doctor was called on the phone to discussed further procedure for her. In the process I had to speak to her in a clear low tone manner and assure her she was going to be fine although she was in pain. Later on during the day, the doctor requested that she should be on telemetry to monitor the heart rate.

 

What did you do well?

What I did well was when I was doing the observation with ECG machine I realised it was not reading properly so I had to esclate to and HCA for support and they noticed that two of the skin tact were not properly stick to the patient skin and I had to rearranged them properly for the machine to record.

On this reflection, I will be discussing about personal care that I supported CPR patient while at the ward for my placement. This patient is bedbound that need her observations to be taken. I introduced myself and the purpose of the procedure explained to the patient. Patient consented for the visit and was supported to stripped wash and dress up. She was always anxious of fall, I had to communicate in low tone and clear manner for the patient to understand and assure her that she was safe. In support with her personal care, I exhibited dignity and respect by closing all the curtains and covering her body parts that were not attended to. Her observations were taken and documented. Although she was always anxious to fall I always had to show care, compassion, communicate and courage and assure her that she was safe and doing well.

What did you do well?

I Communicated in a clear low tone manner for the patient to hear and understand and was very patience with the patient.

What would you have done differently?

Will always try me best to escalate any issues first before it gets out of hand

Today at the center, a patient came in for her vaccine. The team asked her question and she consented to have the vaccine.

This should be her spring booster and she was informed that her vaccine which was Pfizer was not available in the morning and it was moderna that was been administer. She then declined to have it but rather she will come back later during the day for her preferred vaccine which is Pfizer.

The lesson that was learnt here was that, whenever patients are booking their vaccine online the centres that I have provided what vaccine the centre will be given for the day should appear on the system for patient to choose for their own preferences rather for them to turn up and be disappointed.

Practice Supervisor’s comments:

 

The problem should be escalated to NBS to advertise respective vaccines against the centres name so patients don’t have to come later for the vaccine.

Student reflection

 

The angiogram procedure is meant for people with cardiovascular problems The purpose of this procedure is examined the heart and identify any usual activity within the heart and if there is anything that needs to be done to correct the heart movement. A small cut is made in the skin over 1 of your arteries, usually near your groin or wrist and a local anesthseic is used to numb the area so it does not hurt. A long, thin, flexible tube (catheter) is inserted into the artery and is carefully guided to the area being examined and the patient may feel some pushing and pulling when this is done, but it should not be painful. A special dye (contrast agent) is injected through the catheter and a series of X rays are taken as the dye flows through your blood vessels. Sometimes treatment may be done at the same time, such as inserting a balloon or a small tube to open a narrowed artery. This is known as angioplasty.

After the procedure, the catheter is removed and pressure is placed on the cut to stop any bleeding. Stitches are not needed.
