Reflective writing needs some development to help you understand your own practice in terms of what you have got already that is working well & what you are not so good at that you need to work on. You have not adhered to 48 hrs/wk EU Working Time Directive over the duration of the placement – 55.15 hours on 5 consecutive days. This is not a good way to look after yourself or patients! 48 hours is the maximum number of hours allowed per week Limited opportunity for nights or w/e shifts logged on this placement. Peer Feedback

Valerie is a great student, my experience working with her was excellent and she will make a great nurse in future. She was very supportive towards both staffs and the patients and she applies the nmc code to working sector where she remains professional and respect everyone,  I look forward to keep in touch with her and I was glad to work with her.

I was allocated to be at a Renal unit that involves Haemodialysis. This is dialysis unit that provides treatment to in patient and outside patients. I was assigned to be working with the matron of the ward as my Pa and sisters incharge on shifts as my Ps. This gave me a great opportunity to work with all the nurses one to one to help me gain understanding and knowledge about dialysis. It also allowed me to gain lots of patient feedback on my epad or in general help me understand dialysis. This means I had no competition with any student to get attention during my placement duration.

However not having peer feedback wasn’t helpful because I didn’t have feedback from a different prospect. Being on the shifts sometimes was daunting because I was the only student. The first 1 and half I was just observing the marton and nurses some patients where not keen yet to open up to talk to me because I was new. It made me feel very much alone on the ward during that period. But after been trained to use equipment, learning the pre, during and post dialysis sessions, I was able to partake on individual tasks to be part of the team.

I worked with Valerie for a few hours and assessed her caring for patients, she was an excellent communicator and attended to the patients needs with compassion and confidence. She is a great advocator for patients and performs A-E assessments effectively, Valerie is progressing everyday and is becoming a more competent nurse.

Professional Feedback

  • Placement 1

Valerie continues to put herself forward to learn more and ask questions on what she doesn’t understand to improve her knowledge. She has managed to grasp the basics of the nurses duties on the shift and take the initiative to take responsibility to complete tasks on time when required. She is able to liase with other health professionals when necessary and understands the patients progress.

Valerie still excels in learning the kills fast and applying them when required according to the patients needs. She is able to access the medical devices and use them correctly and reporting findings of an investigation.

Valerie is punctual and always in her clean uniform. She able to to the five stages of hand hygiene and donning and doffing system. She always have a good attitude towards patients and the staff. She is always smiling and never complaints. Patients enjoy the days she works on the ward and is always able to showe empty and compassion towards them.

Valerie is an enthusiastic learner with much promise to success if she continues to read and practise. I would recommend her to strengthen her medication administration skills and to educate herself more on the different types of conditions so that she will be able to look after the progress of the patients.

Valerie has an understanding of dialysis patients routine and their treatment plan.

She knows how to line and prime the machine and the importance of setting the machine according to prescription.

She knows the different renal replacement therapies and I have also discussed with the difference between HDF and HD treatment.

She is able to do the observation and assessment for pre and post dialysis as part of their daily routine and able to enter vital signs in the chart.

She is very enthusiastic and always willing to learn more about dialysis patients treatment plan and action.

I can confirmed that for her 6 weeks placement in renal unit she has gained lots of knowledge about the Kidneys disease and dialysis treatment.

Date 16/02/22

Valerie attending the online link lecturer tutorial today. She reports enjoying her experience on the dialysis day unit. She feels she has become part of team and has increased her knowledge and skills around managing renal patients and those requiring dialysis. We discussed aspects that she could do for her EOC and how to utilise a reflection model to help focus her thoughts. She reports she has managed to undertake her medicine management by following a patient and discussing with them about their medication and then discussing these medications with her PA. Due to being the other student on the placement area, peer feedback has been difficult. However, Valerie has written a reflection on the pros and cons of having feedback in relation to this placement, which is a very good use of the disadvantage presented to her.

Valerie spent 4 weeks on the dialysis unit where she was able to directly observe and professionally discussed the various care given to patients during their dialysis treatment. Valerie was able to discuss the functions of the dialysis machines and the different types of treatment such as Haemidialysis, haemodiafiltration and ISO UF. She was able to observe and practice how to prepare and program the dialysis machine and how to strip and clean the machines after their use. The care of dialysis accesses such as Permcath, AV fistula and AV graft were discussed and implemented.

We also discussed the different functions of the normal kidney as well as dialysis principles and treatment.

Valerie was able to demonstrate that she is a fast learner and was able to assist the dialysis nurses to assess patients and deliver the care required.

Valerie was also able to assist the dialysis assistant to prepare the machines safely and effectively.

I have observed Valerie to be honest, caring, well mannered and willing to learn. She interacted well with patients and all staff. She asked relevant questions and was able to take part in many relevant discussions.

Once Valerie has received the appropriate training, she was keen and willing to step in wherever she was able to help. She has a quiet demeanor and was ready to assist patients and nurses at all times.

The student establishes rapport well to patients. She is able to communicate well with the rest of the medical and non-medical team. She has initiative and very keen to learn.

In terms of Knowledge, Valerie manage to learn and experience on how to prepare and administer IV medications and how it is being used for patients. She was able to learn the indication of giving IV medications and the difference from oral administration. Also, she learned how we look after patient’s having tracheostomy like, doing tracheostomy care, suctioning and administering nebulizers. She also learned how does RIG tube works in terms of nutrition for the patient and how they are used in giving medications and enteral feeds to the patient. Valerie manage to apply what she had learned in the University on how to do physical assessments to patients and how to properly document it to the CRS. She was able to successfully apply drug calculations when doing drug rounds in the ward together with his/her practice assessors. Lastly, she manage to interpret NEWS scoring when taking observations to patients and how to learn when to escalate deteriorating patients. Overall, I can say that Valerie was able to gain a lot of knowledge in terms of nursing compared to her 1st day in the ward and we were able to attain all her goals that we have discussed in her initial interview.

In terms of skills, Valerie manage to practice on how to prepare and administer IV medications, how to give Enteral feed and oral medications through RIG tube, learn how to look after patients with tracheotomy, how to do different assessments and how to document them in the CRS, e.g Nursing Notes, Observations, Pain monitoring, VIP scoring, Body map diagram, MUST, Waterlow and RIG care. She laso learned how to apply drug calculations while doing drug rounds and giving medications to patients in the ward together with his/her practice supervisors. She improved her skills in how to take vital signs from the patients and how to interpret the values and escalate the abnormal values to the senior nursing staff and doctor’s. In terms of skills, Valerie gained a lot of experience in performing different nursing skills which improved her confidence and skills on how to this nursing procedures that she can apply when she become a Band 5 nurse. Lastly, I can say that we have managed to attain all the goals that we have agreed on in her initial interview and add so more skills throughout her placement here in Hamble Ward.

In terms of attitude in values, I can say that Valerie always work with the professional boundaries of a NMC nurse and always try to apply the NMC code of conduct when looking after patients and working with the different members of the staff in the hospital. She always has an initiative to learn and tried to help out in the ward as much as possible. Also, she does not forget to inform her practice assessor if she cannot attend her shift and always come in time for her shift. Lastly, I can say that all the good qualities she had shown during her 1st day here in the ward has not change until her last day here in Hamble ward.

Service User Feedback

She took very good care of me. I was very happy that I felt very cared for, listened to and respected. What also made me happy was that she made time to talk to me and understood how I felt.

Did very well with her attitude always smiling to patients. There is nothing the student nurse could have done differently, she was perfect.

Valerie is very caring and nothing was to much trouble for her. She was always very cheerful with very good communication skills and showed a lot of empathy. Valerie was always aware of how I was feeling and acted accordingly, she was always very patient when carrying out any care to a very high standard. I feel that the care I received at all times from Valerie was exemplary and I don’t feel that there was anything that could / should have been done differently. I was very happy that I felt cared for, listened to, understood how I felt, talked to and showed me respect.
