BOOK CHAPTER: Assessing, Planning, Evaluating and Recording Nursing Care for Children and Young People in Partnership


During year 1 this book has helped me a lot by identifying how to give the right needs and support to the patient care. In this book i was given information about the nursing model of undertaking a child assessment, example the model Henderson and roper Logan Tierney activity of living; these models are used to assess a sick child. Reading this 30 mins or a day before the lesson has helped me understand the lesson more and apply the skills and knowledge I have learnt  into my lecture. Also if I need more assistant on my assignment I will come to this book for guidance, It has also helped in my Nursing practice 1, as it gives information on how to physically and mentally assess a child. This book has aided me to increase my confidence in what i found difficult to understand during my lecture. This book is a step by step instruction and information of using the nursing process to help me to assess, plan and deliver and evaluate their care to the child and family.

Furthermore, this book is good recommendations for those child nursing who are struggle with assessing a child needs and health condition, as well as, understanding and building the relationship with parent and yourself.

Reflection-1st placement-January 2023

 1st placements-reflection before the final interview


Student’s self-assessment/reflection on progress: Reflect on your overall progression referring to your personal learning needs, professional values and proficiencies. Identify your strengths and document areas for development.

Knowledge:

During these five weeks on this placement on general surgical ward I have learnt a lot of new things and I have improved a lot since pervious interview with my PA, such as the observing the patients teen of BP to see the average BP range for that patient, if there are any changes or concern it must be reported and recorded, as well as, applying my knowledge of what I have learnt previously.

On my fifth week of placement I have done several Urine analysis, while analysing my supervisor guided me through as to why it is important to do a urine analysis; this is because from urine sample we can find out the following: acidity, glucose, blood and proteins etc. During my past two days of my work placement I have learnt to use the bladder scan, which is used to measure how much urine is your bladder. This is done couple of time to see if the bladder is working alright when passing urine.

One of day during my shift, on the ward we had a serious cardiac arrest and all the seniors had to go to the patient room and once the chest compressions were under control the other staff reached out for the emergency team and gathered the emergency resuscitation equipment. During this time I also approached and helped by grabbing certain equipments the Senior Nurse needed such as the ECG machine and the IV stand, but also had my active listening in place just in case they needed anything else to proceed forward with the situation.

Skills:

Since my previous interview I have Improved more in doing vital signs, taking glucose levels and communication skill has massively increase due to the interaction with my patients and my colleagues. I have also improved and explored the skill of being compassion and showing empathy to patient, this was shown through when my patients would as for something or would ask me to sit with them while they would be expressing how they feel.

I have also developed the skill of being able to adapt myself to a unique days on the ward, such as from caring for the patients to the sudden and unexpected situation you may deal with.

In addition to this, developing the skill of being a strong-minded nurse is very important, this is important skill for me because doing a shift from 7:30 am to 8:00pm is a very long time, so as a nurse it’s important for me to put the patients well-being and emotions before my own. This skill is used everyday because it can effect the care and prioritising the patients needs.

Under supervision one of my colleague, I have learnt the skill of how to give medication and when to give medications, this was a little bit difficult but if I need help or I had questions about a certain medication she would explain the reason why this medication was given or what dosage it must be given. On the same day I learnt how to document few thing under the supervision of the same colleague, I was told that documenting every detail of a patients on Cerner is important because if anything happens there will be previous details to help out with the care plan of the patient if needed. However what I need to develop and understand more of is referral of patients, as I found it difficult doing it by myself.

Attitudes and values:

During this placement I have learnt all different values and attitudes which need to be applied as a nurse. I have developed the core values of being a nurse such as being honesty and consider human dignity while working with my patients and colleagues.Having and developing the attitude of paying attention to details of a patients and empathising with the patient is very important because it gives them a a sense of being valued and listened as you are putting your self in their shoes and trying to understand everything they are trying to say to.

Reflection- Online Simulation

What was the nature of the CPD activity and/or practice-related feedback and/or event or experience in your practice?

I reflected upon my practice of maternity week. During this week we went over some situations that a vulnerable pregnant woman going through domestic abuse, there was a play, and we had a chance where we got to pause the play and changes things around when the the action and communication did not seem right to us, this was in the morning. During the afternoon we had infant feeding and sepsis presentation, during this session we; learnt how to feed an infant and the important of breastfeeding. The sepsis section we had a rapid-fire game where we had question to answer in teams.

What did you learn from the CPD activity and/or feedback and/or event or experience in your practice?

On reflection I felt that I dealt with the play situation in a very calm and professional manner. I feel that I was able to communicate and put my ideas forward. I had to work in teamworks for the second section of the session. It is very important work in a team because in a team you get to share information or knowledge that you have. As Nurse it was very nice to learn that some situations and knowledge of midwifery, as a child nurse it very important to know about the mother of the baby. Improving the knowledge about sepsis is also good because it has made me more aware about the symptoms of sepsis.

How did you change or improve your practice as a result

I realised how important it is to communicate with other people in a team, to understand a certain situation, discussing a situation with the colleagues let you look at the bigger picture. Also, I feel like my management skills and, I think going back home and going over learning more of the knowledge that you gained in very important as it expands your knowledge of certain diseases such as sepsis, because it is a Very dangerous infection.

Having teamwork and and communicate in steam is relevant to the code because it promotes good teamwork and effective outcome for the patients you are going to assess in the future.  This was achieved by communicate with my peers when were put into groups for a presentation, we had to be very wise and relevant to the topic we have chosen and to make sure that everyone was okay with the topic. If a peer could not do or had difficulty in doing an area, we would communicate and help each other out.

What was the nature of the CPD activity and/or practice-related feedback and/or event or experience in your practice?

 During the autism week i have learnt and gained a lot of knowledge about autism. Having to talk through what autism is and how we can support autism was very instreseting. Also one other thing we had guest lectures come in to talk about their experience of dealing with someone who has autism or their family members who had autism, as well as, having a lecture who had autism, it was fantastic and great opportunity to talk to them and talk them. There were days were we had to come in for simulation and they went over skills and three scenarios regarding autism.

 What did you learn from the CPD activity and/or feedback and/or event or experience in your practice?

I think i have learnt a lot fro this week especially improving on the knowledge from my volunteering week with autistic children, it has made more aware of how or what methods to use for those children to achieve their needs and get the help and support they need.

Another thing i learnt from this week was this skill morning i am glad that we went over this because it was a good recap for me, going over the clinical skill such as injection and wound care had tested on what i remember from my clinical practice.  

How did you change or improve your practice as a result?

The clinical skills that i had forgotten some of the steps, going over had been a good recap and good practice day for me, it was a opportunity to open to the lectures to help with what steps or what we have forgotten so they can demonstrate for us.

And put these clinical skill in with autistic actor and then we had to help her out was very good practice as what we have learnt through Theory on Monday we had put that in action, thought i had work with autistic children it made improve more by being aware of my language and body language.

 Learning about autism is relevant to the code because this is where we learn techniques and methods to build a better trust bond the autistic patients, as they do find it difficult to communicate and get along with other then their family members, learning the techies and method from the theory on Monday and then putting in practice when we did the simulation had a very stronger interlink which wa interesting. This then leads to handling the situation professionally which then promotes the professionalism of the nurse or student nurse in clinical practice.

What was the nature of the CPD activity and/or practice-related feedback and/or event or experience in your practice?

During the week of creative therapies week, we went over art, music, dance therapy. We got information about what, how to use creative therapies to help and support patients for their needs and health. It was a very delightful and enjoyable event i came across because as a person who like to enjoy art and music it is very entertaining to see how we can use these creativity in health care needs.

What did you learn from the CPD activity and/or feedback and/or event or experience in your practice?

Having gone through this week of creative therapy i have gained a lot of creative information and ideas to used in my upcoming practice with my patients. Learning the benefit of dance therapy being a connection movement and emotions to boost a patients physical and mental health. What i have learnt the most from this is that through creative therapy patients get to self-express and it can be verbal and non-verbal expression. Also it helps with patients social skills.

How did you change or improve your practice as a result?

I have learnt different dance movement that is used to help patient, i think it has changed a lot the way i think of how to used creative therapy in practice.

I think this week of creative therapy will also help with patient who have autism. And i can use these therapeutic techniques to help and support them.

This relevant to the code because creative therapy is a form of to build a trust bonding with your patient by making them feel comfortable also it is it a way of prioritising the patient for their needs, this is done through them used either dance, art or drama therapy to help them express and have confidence in speaking what they need.
