Samira has demonstrated her ability to support patients when delivering care by listening to patients’ concerns and advocating on their behalf. She raises concerns regarding patients to senior staff members and follows up on such concerns to ensure patients’ needs are met with positive outcomes. She promotes health education by advising patients and their families about diet and exercise, the importance of monitoring blood glucose levels for her diabetic patients, and skin care for vulnerable patients at risk of pressure sore.

Assessing needs and planning care

Utilises relevant knowledge and skills to undertake a comprehensive assessment, continually monitoring a person’s condition, interpret signs of deterioration or distress and escalate appropriately.

Samira has had the opportunity to carry out patient assessments under supervision and has also been able to practice care planning for patients. She has demonstrated her understanding of interpreting signs of deteriorating patients and raised her concerns with senior staff. She effectively carries out patients’ vital signs and escalates any concerns.

Applies relevant knowledge and skills in the provision of more complex person-centred, evidence-based care demonstrating effective communication skills and the ability to document effectively.

Samira has been practising her documentation skills on the Trusts database system (EMS) and has shown good level of understanding in this areas. she understand the need for patients confidentiality and demonstrates person-centred care when supporting patients and their families.

Undertakes relevant risk assessments, discusses risk management and can propose improvements to enhance the quality of care.

Samira understands the need to carry out risk assessments when supporting patients within their homes and how to raise any concerns relating to patient safety. She communicates effectively during staff handovers using this opportunity to improve and enhance the quality of care provided to patients.

Supports the person/persons receiving care and their families in maintaining independence and minimising disruption to their lifestyle, demonstrating understanding of the need for multi-agency working.

Comments

Samira advocates on patients’ behalf and raises concerns appropriately with senior staff. She understands the need for multi-agency working and has had the opportunity to attend MDT meetings including GP huddles to have an understanding of how patients’ concerns are managed to gain positive outcomes.
