Practice Assessor Comments
Knowledge
Samira has shown positive interest in District Nursing and supporting patient within their homes. her knowledge about supporting patients during end of life has improved since she started this placement. She has also learnt about wound management, different types of leg ulcers, grading pressure ulcers and what are some of the suitable dressings that can be used to promote wound healing. Her knowledge of medicine management has also improved and she was able sign off her medicine management competencies on this placement.
Skills
Samira has been practising her skills under direct supervision of the Registered nurse during her placement and has demonstrated her initiative in these areas of practice. some of the skills she has been able to develop during her placement includes; communicating with health professionals, patients and their family by answering and making telephone calls, ordering basic equipment and dressings on the patients behalf, carrying out simple wound dressings under supervision, female catheterization under supervision, documentation, carrying out patient assessment and providing palliative support .
Attitudes and values
Samira has been a pleasure to have as part of the District Nursing team. She demonstrates good communication skills within the team and with her patients. She understands how to present herself as a healthcare professional when visiting patients during home visits and always identify herself as a Student Nurse under supervision prior to carrying out any procedure. She uses her initiative and ask appropriate question and knows her limit as a student. She has maintained positive attitude towards her learning throughout her placement and demonstrates high level of understanding regarding caring for vulnerable patients.

Samira to continue practising wound dressing, researching different types of medication that she may come across during her placements and the different categories of wounds including pressure sores.
Medicine Management (PA Feedback)
Samira demonstrates her knowledge and skills in medicine management. She has shown her ability to administer medication safely and effectively by carrying out the appropriate checks prior to medication administration. Samira uses the BNF to research medications that she is unfamiliar with and ask question about such medication. She understands the importance of explaining side effects to her patients and always signs patients drug chart during visits.

Discusses the possible influences on the person’s/group of people’s mental health and physical health and can highlight a range of factors impacting on them and the wider community.
Comments
Samira has demonstrated her ability to support patients when delivering care by listening to patients’ concerns and advocating on their behalf. She raises concerns regarding patients to senior staff members and follows up on such concerns to ensure patients’ needs are met with positive outcomes. She promotes health education by advising patients and their families about diet and exercise, the importance of monitoring blood glucose levels for her diabetic patients, and skin care for vulnerable patients at risk of pressure sore.
Assessing needs and planning care
Utilises relevant knowledge and skills to undertake a comprehensive assessment, continually monitoring a person’s condition, interpret signs of deterioration or distress and escalate appropriately.
Comments
Samira has had the opportunity to carry out patient assessments under supervision and has also been able to practice care planning for patients. She has demonstrated her understanding of interpreting signs of deteriorating patients and raised her concerns with senior staff. She effectively carries out patients’ vital signs and escalates any concerns.
Providing and evaluating care
Applies relevant knowledge and skills in the provision of more complex person-centred, evidence-based care demonstrating effective communication skills and the ability to document effectively.
Comments
Samira has been practising her documentation skills on the Trusts database system (EMS) and has shown good level of understanding in this areas. she understand the need for patients confidentiality and demonstrates person-centred care when supporting patients and their families.
Improving safety and quality of care
Undertakes relevant risk assessments, discusses risk management and can propose improvements to enhance the quality of care.
Comments
Samira understands the need to carry out risk assessments when supporting patients within their homes and how to raise any concerns relating to patient safety. She communicates effectively during staff handovers using this opportunity to improve and enhance the quality of care provided to patients.
Coordinating and leading nursing care
Supports the person/persons receiving care and their families in maintaining independence and minimising disruption to their lifestyle, demonstrating understanding of the need for multi-agency working.
Comments
Samira advocates on patients’ behalf and raises concerns appropriately with senior staff. She understands the need for multi-agency working and has had the opportunity to attend MDT meetings including GP huddles to have an understanding of how patients’ concerns are managed to gain positive outcomes.

Applies knowledge of healthy lifestyle choices in relation to the person’s/persons’ physical health, mental health and well-being, e.g. smoking cessation.
Comments
Samiratu was able to assess a patient in clinic under the supervision of her assessor. She was professional and worked within her limitation.
Aware of the rationale for capillary blood glucose monitoring and risks of diabetes complications if diabetes is poorly controlled.
Samiratu communicated to the patient using words patient would understand and was able to check her understanding of what was communicated to her.
She is aware that the right information must be provided to the patient to help her make informed decision.

samiratu was able to undertake accurate assessment of the patient with the support of her supervisor.

Samiratu was very respectful about the patient’s condition.

The patient’s safety was maintained, and care was provided according to NICE guidelines.
Referral to the psychologists was recommended to aid patient’s engagement but was delined.

Able to use short and simple words to communicate with patient to avoid misunderstanding and confusion.
Feedback (Link lecturer)
Samira joined the online LL meeting today, she reports enjoying the placement and is maximising her learning through being involved with the clinica; environment of district nursing. The reason for her attendance today was to ask advice about writing a reflection on learning for others. She wasn’t sure who’s email she needed to put in. I have advised she can use the person who she was learning from, however, sometimes this is difficult as the person may not work in the placement area all the time and so she can use her PA’s email details.
Comment (Practice Supervisor)
Samira as we call her was very attentive and willing to learn more about the Doppler test, to detect peripheral arterial disease. Samira showed interest in the procedure and she was keen on observing both legs in and out of the procedure. Samira genuinely wants to improve the patient experience. She was asking intellectual questions regarding the Doppler test and the advantages and disadvantages of the patient having the test done. Because of the great care Samira provided during the Doppler test, the professional took Samira again to the next patient for her to do the test and she did it very well under the supervision.
