Title : Endocrine System / Developing professional relationship in Nursing .

In the Morning Section we learnt about the Endocrine which is the second control system of the body, its hormone travels through the blood and alter the activities of the target cells and also regulates complex processes like growth and development in the human body.

We learnt that the endocrine system consist of Glands which are separated and have no connection to each other , and that they are group of secretory cells surrounded by capillaries that facilitate the diffusion of the hormones from the secretory cell to the blood stream.

We went further to discuss about the Endocrine which produces hormones and lacks dust compared to the Exocrine which produces non – hormone and have dust..

Further more we learnt that Hormone is a chemical messenger secreted by endocrine gland that is  transported in blood and act on target cells in the body.  knowing that the level of hormone in the blood stream is variable. and that the 3 components are Detector, Control centre and the Effector

We discussed about Hypothalamus which is located at the base of the brain linked by the Pituitary stalk to pituitary gland , this is also the major link between Endocrine and the Nervous system which send information to the pituitary gland and the control the Autonomic Nervous system.  The Pituitary gland which is located below the Hypothalamus . Its the size of a pea and weights about 500mg. It also consist of 2 functional lobes which is the Posterior : which is stimulated by the Nervous impulses and Nervous tissues and the Anterior : which is stimulated by the Hypothalamus hormones secreted into the blood stream..

Moving forward , I now know more about the  Endocrine system and how it functions and works in the human body and how it will keep me more informed during my role as a Nursing Associate.

 

Afternoon section

Developing professional relationship in Nursing

In this section we discussed about the Introduction to Multidisciplinary  working, which is described as the mechanism by which holistic care for Patients is ensured. Also we discussed about Interprofessional working , when different professional work with each other and PT to provide integrated social care. Collaborating with is working together to achieve things that cannot be done alone and wee as Partnership working , which is engaging with other professionals .

We discussed further about what the NMC states that all Nurses must be able to play an active and equal role in the interdisciplinary team, communicating and collaborating effectively with different colleagues at all levels , while working across boundaries, actively involving and respecting the contribution of other to be able to integrate person- center care ,whilst knowing when and how to communicate with and refer to other professionals and agencies in other to respect choices of Patients.

We discussed about the benefits of interprofessional working which helps to achieve effective Patient care which avoids fragmentation,  avoid duplication of roles and improve communication. It also combines the expertise of professionals and improve the quality of services . It also enhances mutual understanding and respect of role and cost effective, it also enhances shared decision making for Patients as well as meets Government initiatives.

Lastly we also explored the barriers of interprofessional working , which includes Power and status which often affects relationships and cause tension among care givers ,also some staffs finding it difficult to be a team player, lack of leadership, time constrain and individual personality clashes and lack of knowledge.

Moving forward I know the importance of professional working and how being a good team player can play a major role in providing  a good timely manner care to service users and improve the quality of care we give to Patients as a Nursing associate.

Title : Introduction to E pad

Today in class we had someone to train us on how to use the E pad along side our  Lecturer Colin , we were put through on how to log into our individual E pads and how to navigate the web site. It was an interactive section to know when and how to fill in the e pad as well as the dead lines for it to be done .

We also discussed how to recognise our practice supervisors  in our placements and we also had a chance to discuss the opportunities each student will get to learn from our different placements . The placement will also run for 5 weeks .

I understand I will continue to have support from the University throughout my placements as well as have a named person I can reach out to if any concerns.

We went through how to fill in every aspects of the E pad and also to make sure our practice  supervisors fill in all necessary parts of the E pad relating to them in due course.

I understand I am to make good use of my period in Placement to learn new things related to health and social care, ask questions and make use of all opportunity to given to me to learn new things in my role as a Nursing Associate .

Title: The renal system  and  BLS

The Renal System.

In this topic we learnt about the Renal system , which consist of the Kidney , The Ureters : which carries urine from the kidney to the urinary bladder. The Bladder : which holds urine , it opens into the Urethra and it contains 3 layers which is are the inner layer , the middle layer and the outer layer of the bladder and lastly the Urethra : which is the tube extending from the neck of the bladder to the outside of the body .

we learnt about the function of the Renal system which is the production and the secretion of hormone that stimulate the formation of Red blood cells and it maintains homeostasis of water and electrolytes in the human body. It also helps in the excretion of waste from the body.

we learnt that the Bladder seats behind the pelvic bone and in front of the rectum and that urine is the filtered waste from the blood .

We went further to discuss about the Nephron , Cortex  and the Medulla. We learnt about what urine consist of , which are  water, uric acid , urea, ammonia, sodium , potassium chloride, phosphate, oxalate, sulphates.  Urine is also clear and .amber in colour , and that a healthy adult passes about 1000-1500ml of urine daily. and that the PH is around 6.

Moving forward i have learnt that it is important that blood flow to the renal system is maintained , and that when BP rises the kidney can independently reduce blood flow  and that when a Patient BP systolic drops too low filtration of blood will be impaired and damage to the kidney can occur. and that fluid balance in the body is controlled by the kidneys.

 

BLS.

In the afternoon section we had a practical section in the stimulation room we learnt how to perform compression on adult and children and the amount of rescue breathe to give when the situation arises .

We learnt about the importance of being trained and our responsibility as a Nursing associate.

We demonstrated in group how to deal with situations by calling out the Patients name to check for consciousness, checking for breathe and response , and tilting the head backward while lifting the chin to check for any obstruction in the airway, call for help to be on the way , starting compression and then rescue breathe if safe to do so.

In a child 15: 2 compression and 30:2 in an adult  and also give 5 rescue breathe .

Moving forward I am aware of the importance to keep my training up to date in other to learn appropriate technique  suitable for BLS and the step by step guide on how to deliver CPR to Patients of all ages and how to help deliver this outside my place of work if needed.

Title: A.E assessment / Blood Glucose  and safe use of insulin.

 

A.E assessment and Blood glucose

Today I learnt about the ABCDE way of assessing Patient which consist of assessing the Air way, Breathing , Circulation and shock management, Disability due to neurological deterioration and Exposure and examination.

I went further to learn about SBAR which stands for Situation, Background, Assessment and recommendation.

The ABCDE assessment helps to reveal abnormal physiology  and it principles to perform an ABCDE assessment instigate treatment for life threatening conditions and the need to call for help if needed .

We Air way assessment is to establish the openness  of the airway, to assess the risk of deterioration and ability to protect the airways well as to be able to predict and potential problems.

We can assess the airway by tilting the head, this can be clear, partially obstructed or completely obstructed.. We went further to discuss the conditions that have effects on the airway and the classifications , I read about the upper airway – from the nose and lip to the voice box , The lower airway -between the larynx and the narrow passage of the lungs, the Partial airway, complete airway, acute airway and the chronic airway which is the blockage that takes long to develop or blockage that last long.

Breathing which focuses on ensuring there are no conditions that are impacting the lungs , to assess this we look, listen and feel and we spoke about the common problems which can be respiratory conditions such as asthma, COPD , respiratory depression , infections , pulmonary embolism with the interventions of Oxygen, ventilation , position upright if struggling to breath and specific treatment for example Nebuliser (salbutamol)

Circulation , which can cause a persons BP to drop, it works to transport Oxygen and its not limited to the assessment of the heart . we can assess this by checking the skin colour, BP , Pulse , urine output and the capillary refill time. We learnt about the common problem which can be dehydration, sepsis, shock and cardiac problems .

Moving forward I have learnt how to identify patients with airway obstruction and how to help if such situation arises, I am now able to to put the ABCDE assessment into use and get a better outcome in my line of duty as a Nursing Associate, when to escalate and call for help as well as immediate possible intervention that can be done to avoid deterioration on Patient.

Afternoon session.

The studied about Managing Blood Glucose and safe use of insulin in Patient .

We learnt about the need to take BS is to help guide us in making decision regarding diabetic lifestyle and treatment . To maintain essential control of our BG , also to improve awareness of the symptoms of hypoglycemia and hyperglycemia as well as care for Patient with other life threatening conditions .

We learnt about the  Type 1 – which are insulin dependent diabetes which is the chronic condition in which the pancreas produces little or no insulin itself  and the Type 2 – which is most common in adults which is we the body becomes resistant to insulin or does not make enough insulin .

We learnt about the Blood Glucose  target levels in adult Type 1 – fasting -5- 7 mmol , before meals 4 – 7 mmol and 90 mins after meal 5 – 9 mmol. and in Type 2 ,, before meal 4 – 7 mmol and 90 mins after meal less than 8.5 mmol.

Moving forward I am aware of the difference in the Type 1 and 2 diabetic Patient and what the Blood sugar target should be , I now know which Patient is dependent on insulin and how to care for Patient with diabetes in my line of duty as a Nursing Associate .

Title : Ethical and Legal aspect of Nursing  / Vital signs and News score

Morning section

Today we started by discussing the time frame for our first essay submission and due date for the 14/2/25 , we also discussed  when we are due to sit our first Moc exams which will be taken on campus  sometimes in January 2025 .

We went further on how to write a good Essay and how our Essay should have a learning outcome in relation to the NMC code of practice at all times and to make sure the references are in Harvard forma t and to use Gibbs reflective essay.

We discussed in details the Ethical and Legal aspect of Nursing . We spoke about Autonomy , which is the opportunity given to Patients to be able to make decision for their own care and putting individuals at the centre of their own care at all times. We also went on to look into Beneficence which is doing good actively, by doing what is beneficial for the Patient in a good way by making things much better for the individual we provide care for , we also discussed about Non- Maleficence not to make things worse and Justice which is equality and fairness at all levels in the course of our duty as a Nursing Associate.

We went further to discuss about the NMC code 2018 Accountability vs Responsibility which is acceptance and carrying out a task when delivery care in our line of duty as a Nursing Associate. Furthermore we discussed the Mental Capacity act which enable Patients to make informed decision, we went on to discuss the function of the Mental capasity act which helps to work in the best interest of a Patient, provides the check list of the things to be met as well as the provision of care and or treatment as long as the capacity and the best interest of a Patient as been properly assessed then we can give care to a Patient that lacks capacity .

Lastly we discussed the care model of ethical decision making and the 4 Quadrants that looks into the Medical implication, Patient preference, Quality of life and Contextual factors.

Moving forward I understand the need to give all individual the benefit that they have the capacity to make informed decision regarding their care unless it has been proven that they lack the capacity to do so in my line of duty as a Nursing Associate , and also to make the NMC code section (11) 2018 and important aspect in the way and manner I deliver care as a Nursing Associate .

 

In the afternoon section we had a practical section on how to check Blood pressure manually in the simulation room , we were paired up in 3 in each groups and we checked each others blood pressure and learnt when to leaning to the first beats has the systolic reading and when the beats stops as the diastolic reading . It was a very interesting section and was very informative as well. we made sure we introduced ourselves to each other and gained consent to proceed with care delivery at all times.

Title : Anatomy and physiology of the Nervous System / Importance of communication / Professional values relating to Nursing practice.

ANATOMY AND PHYSIOLOGY OF THE NERVOUS SYSTEM :
In the morning section we learnt about how the nervous system is important in the human body , how it detect and responds to changes inside and outside the human body and its fast response to reactions. We discussed the components of the nervous system, the Central which comprises of the brain and spinal cord as well as the Peripheral which consist of the sensory division and the motor division which splits into Somatic and the Autonomic which then goes further to split into Sympathetic( response to panic /shock : (increased breathing, heart rate ,BP) and the Parasympathetic   which is the response to stimuli.

We discussed about how the Central Nervous system is made up of the brain and spinal cord , and how its acts as the body’s control centre and coordinates the body’s activities and how the brain has approximately 100billion nerves cells and receives information around the body. The Peripheral nervous system on the other hand  is made up of nerves that carry message  to and from the central Nervous system. They both work together to make changes in the body and respond to  stimuli.

We went further to discuss about Neurons which is the basic unit of structure and function in the nervous system. it conduct impulses made up of dendrites( which receives impulses and carry them towards the  cells body and Axon) and Axon ( single extension of the neurons that carries impulses away from the cell body) .

We discussed about the brain and its 3 main sections  . Cerebrum (controls memory, intelligence, language) Cerebellum ( controls balance, posture, coordination)  and the Brans term  . we also discussed the Lobes of the brain and it corresponding function, we talked about the Frontal lobe( personality, intelligence, plaining and problem solving ) The Parietal lobe ( reception and processing of  sensory information from the body) Temporal lobe (memory, emotion , hearing , smell ,judgement ) and the Occipital lobe ( vision input )

We discussed about the brain stem which consist of the midbrain , Pons and the medulla oblongata . The outer layer of the brain and spinal cord which is the Meninges which protects and supports the central Nervous system .and to ensure adequate support of the blood to the Central nervous system  via thousands of blood vessels to produce cerebrum fluid .

Moving forward I have learnt how important the Nervous system is to the body and how the brain works in connection to the entire human body. I know how bleeding in the brain can cause to much pressure and damage the brain , I am aware of the different role of the Nervous system and how it plays a very important role in the human body as the electrical wiring system of the body.

 

IMPORTANCE OF COMMUNICATION / PROFESSIONAL VALUES RELATING TO NURSING PRACTICE.

I the afternoon section , we discussed about the importance of communication in our job role as a Nursing Associate , good communication and the use of effective communication in our role helps us to give person centred care to our Patients as well as being able to pass message between us and the people we care for at all times .

We had a group activity to discuss how upholding the NMC values in relation to good and effective communication and how it protects the Patients and ourselves as a Nursing Associate in our line of duty as a care giver. We discussed about the importance of good and effective communication, being a good listener , being open and honest at all times ,leaving room for questions when relating to our service users , make sure we document for future reviews of events ,simplify message for better understanding of the Patients we care for , as well as know how to safeguard and advocate for our service users when needed.

Moving forward I know how important a good and effective communication is in my job role as a Nursing Associate and how Professional values underpin all aspect of professional practice (Baillie / black 2015) this professional values can influence or attitude both in a positive and a negative way.

Moving forward I understand how communication can be a very difficult barrier and how I can do things better and communicate differently to make communication easier in my role as  a Nursing Associate to the people I care for to have a good and better outcome in my line of duty.

Title : Uniform and named badge ordering / Blood Oxygen Practical section..

UNIFORM AND NAMED BADGE ORDERING

At mid day we ordered our Nursing Associate Student Uniform and Named badge on line via teams with Grahame Gardner , He talked us through how to place our order correctly to avoid any mistakes . We also filled out the online forms for our Hello Named badge .

We later went to the library to get ourselves familiar with the place , we met with one of the library  coordinators  whom inducted us on how to use the library, borrow books and when to return loan books in time . She also showed us how and where to locate book for our Nursing Associate related courses and how to seek help when needed. She explained what our printing entitlement was and how to top up our Croydon College ID cards with more money to be able to print when we run of our free printing

BLOOD OXYGEN PRACTICAL SECTION

In the afternoon section we all went as a class to the simulation class , we were paired up to check each others blood oxygen’s level, heart rate and respiratory rate . we had to do this the professional way  , by first introducing ourselves to each other and gaining consent to check each others vitals signs .

We discussed about how the pulse oximeter measures the blood colour.I learnt that bright red colour of blood is antiviral. If the colour of the blood is  dark that means there is oxygen in the blood for proper blood flow and waste is not been taken away from the blood in the body which makes the blood dark red in colour.

Going forward I now know that the most important vital sign in the human body is the Respiratory Rate.

Title : General discussion points for Apprentices / Bloods / Manual Handling.

GENERAL DISCUSSION FOR APPRENTICES.

Today we had an extensive discussion on how to fill out the tripartite form for our next meeting between the University, Employer and the Apprentice , we also had a brief discussion  on what to tick on the form and filled out the form together in class. we also discussed about some of the things we will be discussing in the meeting .

We also talked recapped on how to fill in our daily learning logs as well as or theory and practical learning logs .

We went further to discuss about the supports available  to us as students , as well as career opportunities available to us after we finish our course and qualify as a Nursing Associates.

We spoke more about the assessment for theory module  which will be due in February 2025, and our first external placements .

I have learnt in this section how to fill out the tripartite forms for the upcoming meeting , learning log and more on how to write my Essays.

 

BLOODS .

In the mid day we learnt about the structure and function of Bloods .

We learnt that bloods carry oxygen around the body as well as sends hormones around the body.

The blood helps to transport gases (Oxygen and Carbon dioxide) , waste product , Nutrient , removes toxins from the body as well as maintenance of core body temperature.

We also talked about the composition of blood, the volume in adult body is about 5l and its made cellular material in a fluid called plasma which makes up of 6 – 8%. the circulating tissues consisting of 3 types of cells , Red blood cell, White blood cells and Platelets. The formation of blood consist of  Hematopoietic : which is the formation of the development of blood cells , The cellular element , in adult produced in the bone marrow and white blood cells which is produced in the lymphatic  tissue and bone marrow . Blood cells need a lot of nutrients to form properly. Examples of the nutrient the blood cell needs are iron, folic acid and vitamin B12.

We talked about some of the diseases associated with bloods like Anaemia which is red blood cell disorder. which is caused by Low red blood cell count, low haemoglobin concentration( iron deficiency) and deformed red blood cell (sickle cell anaemia)

we spoke about white blood cell which is the largest sized blood cells , which is form in the bone marrow and some lymph gland. it is also the primary cell of the immune system, the life style is from 24 hours to several years and it contains nuclei with DNA. White blood cells generally contributes to the immune system.

We went further to learn about different types of blood groups and the 4 major ones consist of  A,B,AB,O.

We learnt about the Cardiovascular system, which supplies  oxygen to the tissues , it transports carbon dioxide from the tissues back to the lungs and nutrients to the tissues of the body, remove waste products from excretion from the body to the kidney and liver and helps to maintain homeostasis.

 

In the afternoon section we went to the stimulation room for practical section, we washed each others faces in pairs and reflected on the the way we felt being cared for and relate it to the way we will care for Patients in our job role as a Nursing Associate, also we went further learn how to wash Patients in bed and reflected on the way we attend to Patient and how to manoeuvre with the help of the Manual handling training.

We later also had a practical  section on using the hoist and the step by step of how to hoist a Patient from a chair safely , it was well explained in details and it was a very interesting section.

Moving on from what i have learnt today, I am able to do my theory and practical learning log, more confident in attending to Patient during personal care and safe manual handling of Patient in their vulnerable state.

Title: Respiratory System / Introduction to academic writing

RESPIRATORY SYSTEM:

Today we discussed  about the respiratory system and how it functions in the human body .

The respiratory system function to provide our body with oxygen and remove carbon dioxide  , oxygen is required by our cells to make energy and this is brought in through the lungs  and delivered to the cells by the blood stream. I also learnt that Carbon dioxide is carried away from from our cells to the lungs and this is expired from our body through the lungs.

Further more I learnt  about the respiratory system consisting of the Thoracic cavity, upper and lower respiratory tract and the Lungs, the muscles of the respiratory system which consist of the Diaphragm that plays one of the major role  in the respiratory system , it contract to allow for oxygen inhalation into the respiratory system through the nose which filters the out the air we breathe in ,or the mouth into the lungs. And these turns into waste product in our body (carbon dioxide ) which is expired from the body through the lungs.

Moving forward putting what I have learnt about the respiratory system into practice ,I now have more insight into the respiratory system of the human body and I will be able to implement this in my work role,  knowing the importance of free air way for oxygen passage into the lungs is now more clearer and I am now more confident about using what I now know into my job role , also  my training in Basic life support  as a fundamental training skill will be put to use in a more reliable way. I will also be able to identify and support people with respiratory distress.

 

ACADEMIC WRITING

In the afternoon section of the day , we learnt about Academic writing level 4.

we learnt about how to write essays , and how to apply the Bloom’s Taxonomy , which give us the guideline of how to write a good essay by following the pattern: Create , evaluate, analyse, apply, understand and remember.

I also learnt the importance of referencing and how to use it in my essays writings. I also learnt how to proof read my essays as well as use references at the end of each quote ,and not at the end of the essay. Also in this section I learnt how important it is to use the full word Nursing Associate when writing my essays rather than using abbreviations like NA in essays.

Moving forward I have learnt how to write essays better in a more reflective way  , also use full words rather than using contractile English, also to use own words and not direct words from a third party.

Also the use reference DIO number as the quote can be easily traced and remain in the internet space , instead of the full source of reference which can not be trace again in the future for verification of reference

Title : Developing professional values and behaviour / Anatomy of physiology  , System, Cells and Homeostatic.

DEVELOPING PROFESSIONAL VALUES AND BEHAVIOUR

In the morning section we learnt about the NMC code of conduct which contains the standard that Nurses, Midwives and Nursing Associates must uphold in direct PT care and leadership research and education.

I also learnt about the NMC 6Cs which comprises of Compassion, Communication, Competence, Courage, Commitment, and Care , including duty of candour and how we are bind by the NMC to implement them in our role as Nursing Associate.

We went further to discuss the British Values which we need to adhere to in our profession and how to implement them in our dad to day role . Also we discussed about what is expected of Nursing Associates in our job role and to know our limitations on what we can and cannot do as a Nursing Associate.

Moving forward I have learn what is expected of me as a Nursing Associate and how to implement the NMC 6Cs in my day to day care delivery as well uphold and promote British values in my line of duty, and also apply duty of candour in my practice.

 

ANATOMY OF PHYSIOLOGY : SYSTEM, CELLS AND HOMEOSTATIC

During the afternoon section we discussed about the body system and cells and the concept of homeostatic. I also learnt about Anatomical positions which tells us about our body positioning and language when dealing with Patients in our role as a Nursing Associate.

we went further to discuss  the Anatomical positioning which  includes , Standing upright, Forward facing, arms at the side, Palms forward, Feet parallel as well as knowing Patients positioning from the right and left positioning when dealing with people in our line of duty. we also discussed the level of organisation of the body which includes the Chemical level, Cellular level, Tissue level, Organ level, Organ system level and the organismal level.

We learnt about Zygote , which is formed when the ovum and spermatozoon fuse, and that Normal cells know when to divide to create new cells, repair or die.

We discussed about Homeostasis which is described as the body’s ability to maintain relatively stable internal body conditions . we went further in this section to discussed  the 2  examples of Homeostasis mechanism which is Blood glucose and Blood pressure.

Moving forward I  know the 5 essentials  of survival in the human body which is water, temperature, nutrients , oxygen and pressure as well as the types of  skin cells that can become cancerous . Also I know how to response  to hypothermia . I will also put into practice the Anatomical body positioning  in my line of duty as a Nursing Associate.
