There is some understanding of COPD but this could be improved.

 

Discussion and analysis

The introduction presents what the essay will contain. The pathophysiology of COPD is not discussed in depth. Some of the focus is out of context. Assumptions were made which were incorrect. Mobility was identified as the area to focus on but some of the assessment tools chosen were irrelevant in Derek’s situation (see comments I made within text), Derek’s cause for reduced mobility has not been fully considered. More thought and compassion is needed when considering Derek’s situation, this is lacking. Self-management is mentioned but some of the strategies are already being implemented by Derek and his wife. You were able to explain the nurse’s impact in Derek’s life. The conclusion could have been improved.

Evidence of reading is limited as there is reliance on websites as references.

 

Academic style (including referencing, structure, grammar)

There is no consistence in reference style and some literatures are incorrectly referenced. The sentences were not linking at times. Please seek guidance from the Academic Achievement team to improve in academic writing skills. There is a need to improve in grammar and presentational skills.

 

Summary

You have addressed the learning outcomes, however there is a need to improve in attention to detail and making sure you understand what is required of you from an assignment brief. Refrain from writing out of context. More breadth and depth is needed in the work. Please consider the comments I made within text. The word count and similarities are acceptable.

I had the opportunity to work with Patrick at Dalby ward. I noticed that he is a hard worker with a high quality of professionalism. Patrick compassion and care for patients has been amazing.

He has a good communication and he able to interact with multiple disciplinary.

i have no doubt that Patrick will be a good addition d’or the nhs .

Patrick is showing a positive approach to learning new skills and trying to exercise it properly. As per my observation, he is a good listener and developing new skills and making significant progress in clinical practice. He is showing good communication with patients and has good interaction with the team .Patrick is managing the proper way to give medications and helps for preparing it and he has a good knowledge about how to manage challenging patients .He is well-behaved and will be an asset to the Nurses team.

Today I observed Patrick prepare and administer medication during medication round with supervisor.

  • Patrick was able to explain what medication was being prepared and why it was being administered.
  • Patrick was able to complete the safety checks and state the patient’s rights without prompting.
  • Patrick was able to document the type of medication, batch number and expiry date.
  • Once completing all checks, Patrick approached the right patient, again complete the 7 pt’s rights, gained consent and explained the procedure with clear communication and was able to reassure the pt.
  • Patrick demonstrated excellent sub cut Injection technique and correct sharps disposal without prompting.
  • Patrick’ demonstrate compassion and excellent communication skills with not only patients but also with staff.
  • further development- to continue practising technique.

Medicines management is essential for patient safety, treatment efficacy, adherence, patient education, and continuous monitoring. It plays an important role in the healthcare system.

As a third-year student, I felt anxious about the thought of medication. I wanted to know the types of medication used in this particular ward( Dalby). I realised with pleasure the range of medications used such as blood thinner medication, tablets to reduce or control a patient’s cholesterol, tablets against seizure, and medicine to regulate blood glucose levels.

I was afraid to make mistakes and put the patient’s life at risk. However, after a few shifts, I started to practice confidently under supervision. I was able to explain the medication I was giving to the patient and its effect. I was able to prepare IV medication and explain the reason it was given. Also, I was able to document the types of medication, batch number, and expiration date. Ensuring patient safety was essential as it was necessary to apply the 7 patient’s rights which are rights ( patient, medication, dose, route, time, reason, documentation ) before administering it to them.

At the end of this placement, my Medicines management has improved and I feel this has helped me feel confident to work independently throughout the NHS in future

I worked at Dalby ward which is a dementia and delirium ward. Throughout my placement, preserving patient safety and their environment was essential. When I started my placement, it was challenging and I worried about patient overreactions. I learned that in this ward, I must be aware of various cognitive issues patients are facing as some can become verbally or physically aggressive. My duty was to ensure a safe environment for the patient, their family, and the whole staff. To preserve my patient’s safety, I had to ensure that the bed rails were always on; this would stop them from falling from the bed. I also ensured sharp staff such as needles were disposed of in the sharp bin.

During a shift, I was also able to raise concerns to the nurse in charge as when I was doing my routine check of EWS, I noticed a patient’s saturation had deteriorated.

Moreover, some shifts were more exhausting than others as some Dementia patients became agitated and walked around the ward. To avoid any incident I had to reassure and calm them down under supervision. I was always working within the limit of my competence as my supervisor and other staff nurses were always supportive. Working in this ward, I was always available to support any personal care or any other task.

In the end, this was a great experience in nursing as my patient’s safety was always prioritised and I gained more knowledge and skills during this placement.
