There is a lack of familiarity of the nursing process and your work is not referenced, which is required to achieve this learning outcome. There is evidence of application to practice of the nursing process through use of the case study. There is a lack of clarity in your work and some of the statements you have made are too vague. Follow the guidance provided regarding the assignment. See my annotations.

 

LO2: Demonstrate the ability to collect relevant patient data through a holistic nursing assessment.

There is a lack of familiarity with this learning outcome and as a result you have not demonstrated sufficient knowledge, there is a lack of discussion and analysis. There is evidence of use of the RLT ADLs in assessment section of the case study patient, however this is underdeveloped and superficial. Your discussion around assessment tools led me to believe you do not understand how and why they are used. You must be clear with your intent. See annotations.

 

LO3: Demonstrate the principles of planning care through shared decision-making LO4: Identify the key components of high-quality person-centred care.

Knowledge, understanding and application to practice are not demonstrated with sufficient discussion and analysis. There is evidence of understanding of shared decision making and patient centred care in the care plan discussions. The way in which your discussion is structured makes it difficult to demonstrate your understanding. Whilst you are aware of Roy throughout simply using the nursing process does not ensure person centred care, just as assessment tools do not prevent person centred care.
