I was assigned patient  M who was due for an ankle wound dressing 06/12/21. On gaining entry to her house, I introduced myself by stating my name and explaining that I was a student nurse. I went on to gain consent  if she was happy for me to change her dressing under supervision or if she would prefer it to be done by a qualified nurse. After she consented, asked if I can wash my hand and she directed me to the bathroom. I first read the care plan and then proceed to gathering all the items I needed such as sterile pack, dressing and saline solution.

Before I start , I explained to the patient that I was going to remove the old dressing , clean the wound and redress it with a new dressing and bandage the leg to promote the healing
When I started removing the dressing, I kept checking if she was alright or not . As she was fine, that encourage me to proceed. After cleaning the wound, I redressed it and carry on  with the bandage. When I finished, I asked her if she was happy with the dressing but she told me that the confifast was too tight and that she was uncomfortable. I asked her if I should remove it and she agreed to it. I then removed  it after consultation with my supervisor .

On reflection, I acted in accordance with the NMC code (NMC,2018) in  showing respect to the patient by thanking her  for letting us in her home . By  gaining consent before changing her dressing, I respected her right to consent or decline any medical treatment known as right to autonomy.

Throughout my placement, I have demonstrated my professionalism by being always on time and being courteous and polite with not only patients but also colleagues.
