Learning Log on TNA Supernumerary Shift 22/05/25

What was Learned:

Being able to join ward rounds and case reviews with the Charge Nurse and the multidisciplinary team(MDT).

Having an in-depth perspective on what happens for new admissions and sorting out how to aid with discharge reports and what the MDT can provide support. This can either be a therapist can join a patient during their discharge and can do a final assessment during discharge on how to properly tranfer and make the patient independently support themselves within their own home.

The Ward rounds with the doctors has helped create an understanding of how they can provide support and immediate changes to the patients medications and the doctors providiing some medical insight on how to support the patients and how it can be implemented it into the ward level through the Nursing team.
