Outreach to Infection and Prevention Control

I attended my outreach today on my last day of placement on Infection and Prevention Control

There was a short orientation of 20 mins starting from 8:30 am with the Infection and Prevention Control compliance worker in regards to the purpose and compliance of the audit (due to post Covid)

I participated in going through six wards within the organisation that needed to have an audit being done

I participated in being able to do one audit and following the thorough processing method and interacted with the Charge Nurse of the ward for the day and discussed the feedback from the audit process

The entire outreach session finished at 12:06 noon

Episode of Care

Supported a patient today who has some challenging behaviour during personal care

I had helped assist with redirecting the patient when they started becoming agitated

I helped support the patient and the carers with complying with the standards of care with maintaining privacy and dignity for the patient

I complied with the organisations moving and handling protocols and had support from the carers

Participated in a Compass Session

These are the things that I have learned when I have observed joining a Compass session with a patient.

Supporting Brain Injury service users who have a capacity and who are under DOL’s, in doing leisure based activities. The group of patients being supported don’t have any activities during the day.

The computer room uses a variety of different types of technology such as computers, touch screens and sensor based technology to help with executing simple-complex tasks. This depends on the level of movement of the patients and can help improve their hand-eye coordination overtime.

The purpose of the sessions help patients with a variety of acquired brain injury that focuses on their goals of managing their mood, managing patients with challenging behaviour and supporting patients who want to enjoy watching cultural based videos. This helps to ensure that any service user attending the session is accommodated to the patients preference.

The computer room also holds a space to do assessments for how patients quality of life can improve with Augmentative Assistive Devices which can help in the patients everyday life. Members of the multi-disciplinary team are present to participate in order to help collate information from each speciality such as Speech Language therapists and Occupational Therapists and more.

Volunteers are present when supporting patients, in order for service users to engage with them which helps increase their mood and morale in doing the activities.

Episode of care

Supported a patient during a personal care episode

Following the patients moving and handling guidelines and working with one of the healthcare workers

Completed the entire personal care episode and providing and supported the patients privacy and dignity. Communicated effectively the different steps in order to engage with the patient and let them be involved with their physical hygiene

Properly dressed the patient and appropriately transferred them to their wheelchair and gave oral hygiene.

Outreach Reflection

Learning Log during outreach on Ventilator Long Term Ward

I have learned how to document and reflect the data from ventilator displays into the electronic patient record

Being provided knowledge on the steps of supporting the patients with invasive ventilation to connecting the patients to different humidification tubes for assisted breathing and also supporting them with trachea cuff down. In which the trachea cuff down involves two people as there should be a person responsible in suctioning the free flowing secretions and another person managing the cuff down.

Being able to know the purposes of supporting patients during emergencies through the  Aenesthetic Mapleson C ambu bag being within reach to support patients in emergency situations. Following the designated protocol within the ward setting on my outreach.

Being provided the knowledge on doing routine cough assist  for regular clearing of airway for patients. This improves their breathing and saturations. Most of the patients do experience orthostatic hypotension in which are supported with medication and equipment (abdominal binder, TED Stockings)

Following routine changing of equipment i.e swapping devices such as ventilators, changing the humidification chambers to avoid build up of any bacteria following infection control protocols. Knowing when to change the humidificaion filters to decrease the risks of chest complications i.e build of thicker secretions, prone to chest infections i.e pneumonia

Working alongside a Registered Nurse in providing clinical care and doing drug calculations and administration of medication. Supporting patients with buried bumper checks for PEG Tube patients before administering medications through PEG tube.

Being able to support the Registered Nurse with doing wound care management on a patient such as placing some perscribe/ reccomended dressings before applying the trache tape around the patients neck. To help maintain the patients skin integrity.

Knowing the importance of having routine bladder wash out for urethral catheter patients as this helps decrease the chances of cathether associated infections.

Title: Learning about hwo to access the EPAD

Date: 23/05/25

What?

So what?

Learning about how to properly do the EPAD and how to follow the standards in doing the level of work. This should also include doinng draft work and offciailly sorting out the infomration being inputted and then get an approval from both the practice assessor and the practice supervisors and this can help with properly vaidating the infommration being placed and how this will be evident for the placement.

Doing the practice hours as well and following the policy. The hourswill be co-signed by the assessor and their signature will be their work email to show that it has been approved.  There will be a delay as well when it has fully been filled in, knowing so will avoid duplicate entry.

Proficiencies are amndatory to be signed off by an assessor or supervisor

Now what?

Learning about how to input the data on the EPADs during the different placements within Year 1/Part 1 this can help with a general understanding of how to properly document accurate information while having the proper verification checks made by the Placement Assessor and Placement Supervisor.

Title: Simulation

What?

Having a simulation of a ward based setting

So what?

Performing the simulation and doing a debrief of what were the appropriate tasks at hand and what is needed to be reflected on in terms of patients confidentiality, safety and what to do as a Student Nursing Associate within the simulated ward setting

Now what?

Having the simulation exercise has helped develop an understanding on the safety of the patients wellbeing. There are known challenges of breaching confidentiality if there have been no proper checks for an unknown personnel asking for a patients condition. This is pivotal to the overall safety of a patient.

What was Learned:

Being able to join ward rounds and case reviews with the Charge Nurse and the multidisciplinary team(MDT).

Having an in-depth perspective on what happens for new admissions and sorting out how to aid with discharge reports and what the MDT can provide support. This can either be a therapist can join a patient during their discharge and can do a final assessment during discharge on how to properly tranfer and make the patient independently support themselves within their own home.

 

The Ward rounds with the doctors has helped create an understanding of how they can provide support and immediate changes to the patients medications and the doctors providiing some medical insight on how to support the patients and how it can be implemented it into the ward level through the Nursing team.

Title: Digestive System/Introduction to what happens during Placement

What?

Learning about the digestive system and learning about what will happen during the placement

So what?

Learning about the different parts on a micro level that are involved in the digestive system. There are different parts of the G.I. Tract and how the entire process helps with the absorption of nutrition and how the body eliminates the waste products in the digestive system.

Now what?

Learning about the different types of the digestive system as a Nursing Associate. Has helped me undergo a deeper level of understanding of the different parts that are involved of the digestive system. This helps create an awareness on how to support patient groups if ever, a part of their digestive system is being to impacted and how I can support my patients.

Title: Clinical Skills- A-E Assessment

What?

Learning about how to assess patients using the A-E Assessment

So what?

The A-E Assessment helps with looking into the:

Airway of the patient- is it blocked? Are there any complications??

Breathing- what is the patients Respiratory Rate, Blood pressure and what does this indicate on the NewS2

Circulation- Doing a blood sugar test when needed, to show if the patient has low blood sugar or high. Blood gas using the 5 second hold on a finger and seeing a 2 second reaction when colour comes back. If it takes more time for the colour to comeback, the patient has higher level of CO2( Carbon Dioxide)

Disability- does the patient have any medical history. Do they have any health issues that can be easily noted, during an assessment that can help pinpoint a general idea of the patients presentation

Exposure- what can we escalate and inform the Nurses and Doctors to help with the patients well being and quality of life.

Friends or Family- during the incident with the patient, do they have family with them? Do they need their family or friends to be contacted in an emergency? Are they aware of the patient/clients condition and how this can be relevant to pinpoint how to support the patient

Now what?

As a Nursing Associate, learning about how to do an A-E assessment helps with creating an understanding on how to support a patient and how to develop the critical thinking skills when working with the Nurses.

Title: Immune System (Self Directed)

What?

Learning about the different parts of the immune system

So what?

Learning about the different parts of the immune system. The parts are able to help filter out toxins and help build immunity for the body

Now what?

As a Nursing Associate, learning about the Immune System has helped create an understanding of how to support patients when they are undergoing an infection and how I can help treat them.

What?

Performing as TNA under supervision with Registered Nurse
So what?

Learning about how to do nursing summary notes and doing appropriate observations on patients.

Now what?

The learning that I can get is to focus on accurately documenting and if a future incident of a wrong entry has been made,, do not panic and ask for support. Not only that, but double check, triple check and continuously check before inputting an observation entry.
