Student reflection on an episode of care
Within your reflection, describe the episode of care and how you assessed, delivered and evaluated care.
On 18th of March on my day shift, I met a patient who cannot do personal care by himself. He is an NG feed and his mouth was coated. I assessed the need for mouth care and I was given opportunity to support a patient to do oral care. I introduced myself and gained consent. I gathered all equipment that are needed for the oral hygiene and I maintained the patient privacy and dignity by closing the curtains.
What did you do well?
I gain consent from the patient before giving the oral hygiene by introduce my self, I also close the curtain to maintained is privacy , I also raised bed to maintain is health and safety after oral hygiene the was excited.
What would you have done differently?
I should have gathered all the equipment for the oral hygiene to save more time and to reduce the risk of any necessary to leave patient alone in other to look for equipment such as brush, water toothpaste and towel i should have prevented the risk by living the patient alone an unattended.

Stella identified the need and importance of oral care for the patient. Also, she done an assessment through which she found out the patient’s level of consciousness and cooperation.

Comments
Stella always values patient’s privacy and dignity. She provided privacy and dignity by closing the curtains. Also, she maintained comfort by providing a sitting up position.

Applies the principles of health promotion and improvement as appropriate when caring for the individual and their families.
Comments
Ste understands that regular mouth care is important to avoid oral infections and respiratory tract infections. Also, she knows that poor oral hygiene can lead to poor oral intake.By providing oral care she applied the principle of health promotion in patient care.
Improving safety and quality of care
Undertakes relevant risk assessments (e.g. falls, skin integrity, mental capacity) that may be required and demonstrates an understanding of the difference between risk aversion and risk management.
Comments
Stella done an assessment through which she gained an idea about how patient can follow her instructions and what is the understanding level of patient. Also, she provided a upright position to prevent aspiration and with bed rails to prevent falls.
Coordinating care
Utilises a range of communication skills to effectively engage with the person receiving care, their family/carers and members of the multidisciplinary team in the provision and evaluation of care.

Stella explained to patient and his wife about the need of oral care and how they can help her in the procedure.
