REFLECTIONS FROM PLACEMENT IN MEDICAL WARD( B5 FRAILTY WARD)

I have progressed well in the clinical area that I was working. I have been assessed and have achieved sufficient number of skills such as taking of the vitals signs and to record it. If below/above normal range to report to the nurse in charge. To do catheter care when emptying the catheter bag and dispose it properly. Working in Frailty ward, I have had an opportunity to learn about Frailty, such as what Frailty is and how they assess patients using clinical Frailty score and I have learn more about Frailty by completing e-learning. I have maintained professional values all the time that has helped me to receive positive feedback from the staff as well as from patients and met all the professionals values.

I feel confident carry out tasks, such as taking vitals sign, personal care, administration of medication under direct supervision which I have had plenty of opportunities to practise and demonstrate it. Worked with the team and follow the trust policy and guidelines all the time

Practice administering subcutaneous injection such as Dalteparine injection, Heparin, insulin. Carrying out head to toe assessment on admission of new patient to the ward. Getting involve in discharges such as making sure that patient has got their TTO’s, next of kin is informed and aware about discharge, cannula is removed prior to discharge, transport is booked and if patient lives alone to make sure snack box is sent with them. Using of PPE and hand washing all the time before and after procedure such as personal care, administrating medication, before and after touching patient.

I have improved my active listening skills and remembering to report the concerns of the patients and esclate to appropriate member of the staff. I have always act in the best interest of the patient all the times. Always listened to the patient and respond to their preferences and concerns. Always treat people individually with kindness, respect and compassion and uphold their’s rights. Recognise when service users are anxious or in distress and respond compassionately and politely and work under trust policy.

Stimulation Practice Learning Placement( Focus On Autism)

Whilst on this Autism stimulation placement, I gained a lot of knowledge on Autism which is a neurological and developmental disorder that affects people to interact with others. I also learned that not all people with learning disability will have Autism or vice versa. Autism people may have repetitive behaviour such as repeating words or phrases, they may be more sensitive with such as bright lights or sounds.

During skills and stimulation , I got an opportunity to meet with an autistic patient as a service user face to face and got experience to work with her. It was really a good opportunity for me as I haven’t experienced before working with autistic patients. I learned some very good skills on how to with autistic patients. From that stimulation practice skills, working with learning disability and autism patients, I am going take that always communicate is very important. Always communicate clearly and slowly and be a good listener to them . Give some space and time to reply them back and always be no judgemental.

During my Independent self study , I did learning on Autism Awareness. It was a very heart touching story. Also, I got an opportunity to learn and gain a lot of knowledge about Autism. In the future, if I get an opportunity to work with learning disability/ autistic patients, I am confident to work with the knowledge I have gained through stimulation practice and from e- learning from health.

During my skill practice placement, I got another good opportunity to practice IM injections, subcutaneous injection, Aseptic Non Touch Technique wound dressing and measure manual blood pressure. I feel more confident to do all this skills and will apply my knowledge in clinical area if I will get a chance to show my skills.

Stimulation Practice Learning Placement( Creative Therapy)

Whilst in this placement, I learnt that it is interpersonal communication process where people exchange their feelings, information and meaning through verbal and non verbal communication. It is not just about how it is said or done but is also about what is been said whether it is through dance/drama/art/music therapy. This creative therapy is also helpful for changes to our mental state, mind and body connection to each other. this creative therapy is also a process of means of exploring feelings and experiences.

As a result, working in clinical/community setting, I will try to improve communication/ gain trust by using different types of creative therapy with service users that I have I have learnt in my stimulation practice. It will help me to overcomes with barriers if there is any and will also help me to gain confident in myself and work throughout with courage, care, compassion, competence, commitment and communication.
