June232023: First placement experience

Date of Learning: 23.06.2023

Time: 9:30 to 16:30

Title of Learning Activity: First placement experience.

I shared my experience in my first placements with the class. I am very pleased and happy as I found everyone in my first placement was very friendly and accommodating. It was a completely different setting compared to my ward. I met my Practice Assessor and Supervisors and nominated them on my ePAD. I completed the placement orientation and first initial interview on time. I was allocated to learn in the recovery room and learned a lot from highly skilled nurses.

On admission, I observed completing the pre-operative checklist. I learned to assist patients in preparation for surgery and conduct some tests (e.g. blood sugar check, urinalysis, checking patients’ vital signs, and educating patients in wearing compression stockings to prevent blood clots developing in the leg, which is known as deep vein thrombosis (DVT).

Recovery: I learned from recovery nurses when they received patients who had just come out of surgery and were under local and general anaesthetic. They used the SBAR tool and A to E assessments and constant monitoring. Offering patients something to eat and drink and monitoring bladder and bowel health.

Discharge: Patients were carefully monitored and assessed, educated on the post-operative plan, details about medication to take home, and given more information in case of emergency after discharge.

My other classmates also shared their experiences and we were given information on who to contact if we encounter any problems with our ePAD.

(Itemise learning activity and reflect on the main points of learning from each. You should identify for each entry the relevant KSB)

Reflection on Learning Activity (include model of reflection e.g. Driscoll or Gibbs):

What?

Lecture about Equality, Diversity and Inclusivity

Now What?

We had a discussion about equality, diversity and inclusivity.

Each and everyone were asked to define equality and it seems there was confusion as everyone defined it as treating everyone the same or fairly but what stuck in my mind was it was defined as equal opportunities in your fullest potential.

Equality definition is making sure people are treated fairly, meeting individual’s need appropriately and challenging the factors that limit individual’s opportunities.

Three pictures was shown and each one shows different ways to treat each individual but the last picture was the best one, it removed the barrier so everyone has the chance.

Diversity was discussed and each of us gave our definition of it.

The importance of knowing and being aware of it.

The following were highlighted:

NMC Code (2018 1.3  to void making assumptions and recognise diversity and individual choice. Examples were given.

  • NMC Standards of Proficiency 1.14 provide and promote non-discriminatory, person-centered and sensitive care at all times, reflecting on people’s values and beliefs, diverse backgrounds, cultural characteristics, language requirements, needs, and preferences, taking account of any need for adjustments

Inclusivity was also included in the discussion, and this is where came my given example.

For example: if a meeting will be in a venue and among the participants was a person with a disability like using a wheelchair. The people who arrange the meeting should consider a venue where there is access to lifts or ramps.

The protected characteristics were mentioned under The Equality Act (2010) which cannot be used as reasons to treat people unfairly. These are the following:

  • Age
  • Disability
  • Gender-re-assignment
  • Marriage and civil partnership
  • Pregnancy and maternity care
  • Race
  • Religion and belief
  • Sex
  • Sexual orientation

So what?

This lecture helps me thoroughly understand what discrimination is. It reminds me what the things I experienced that I thought were okay but after the discussion it made me realise some are not.

Understanding equality, diversity, and inclusivity is easy but knowing the real definition could be tricky as all got similarities.

Now what?

This lecture will help me apply my knowledge and improve my skills in knowing what is fair and what is not and be mindful especially to individuals who are in protected characteristics.

What?

Introduction to using EPAD

The responsibilities of a learner were mentioned.

We will have Practice Supervisors, Practice Assessors, and Academic Assessors. Their role and responsibilities were explained. The importance of their role in supporting and guiding us as learners?

The following documents are designed around the following component of assessment and feedback.

  • Initial, Mid-Point and Final Interviews per placement
  • Professional Values
  • Proficiencies Episode(s) of Care
  • Medicines Management
  • Patient/Service User/Carer Feedback Form
  • Record of working with and learning from others/interprofessional working
  • Record of communication/additional feedback
  • Record of peer feedback – PAD 2
  • Ongoing Achievement Record

On orientation, it was explained what to do and what needs to be completed and signed on the EPAD.

And on the first week of placements, there are documents that must be completed. The dateline will show on EPAD.

Professional values must be upheld throughout.

So What

The EPAD will be my tool to use in providing evidence of what I achieved and completed for my placements.

It will give me guidance on what to do and needs to complete each week.

Now what?

I will use the EPAD in making sure I comply with all the evidence-based learning and on top of each document to complete for my placement.

Reflection based on Gibbs reflective cycle (1988)

What?

Lecture about Nervous System

The following topics were discussed:

o   Functions of the Nervous system

o   Structural classification

o   Central nervous system

o   Peripheral nervous system

o   Human nervous system and neuron parts

o   The internal structure of the brain and 12 cranial nerves

o   Meninges and the spinal cord

We had an activity where we wrote about five neurological disorders, identifying the causes and effects on a person’s physical and mental well-being.

I wrote about Multiple Sclerosis, epilepsy, Parkinson’s disease.

It highlights the following in general: headaches, seizures, Parkinson’s disease, dementia, and disorders of the nervous system.

So what?

Before the lecture, I watched videos and read about the nervous system to gain a basic understanding. The lecture helped me understand the nervous system, but I don’t think I understand it fully as it is such a complex topic that I think I need to read it through again or watch movies to understand it more.

Now what?

I will read the slides again to understand them and maybe watch helpful videos to understand them more. As we go through it, I should make it a priority to fully understand it as part of my learning.

What?

Lecture about the musculoskeletal system but the

The lecture focused more on the application of musculoskeletal injuries to practice.

The lecture discussed the following:

  • addressing musculoskeletal injuries
  • history-recent trauma and past medical history
  • physical examination
  • classification and management of musculoskeletal injuries with pictures

So what?

The musculoskeletal system was another big topic I struggled with understanding. I watched videos and slides for the lecture, but I still did not understand it.

However, when we discussed the injuries, I remembered my previous trauma patients from our unit. Though I have not seen the real injuries of the patients on my ward, this gives me an understanding of what the injuries looked like before they were moved to the trauma ward. The example pictures though not pretty, help me understand it better.

Now What?

I will need to read, learn and watch videos of the musculoskeletal system to get a more understanding of its parts and importance. For learning and understanding, I’ll observe patients who have the same injuries.
